did someone encoutner the issue that you dont see a checkbox in even if you applyed the checkbox element?

thank you for your report. Sadly, I can’t reproduce the issue on my local install. Would you mind sending a screenshot of the form settings - especially that checkbox field that is missing?

Hi @lev,
as far as I see, you don’t have anything inside the “Options” field. Add (at least) one line, and the checkbox should appear. 
Inside the Value you set which checkboxes are checked by default.
