Human validator form field with question and required answer

Hi! I would like to have a form option that requires visitors to answer a question in order to submit the form. This would be string-based, so it could be a number or a text answer.

Example questions:

  • Q: “What is eleven minus two (in written form)?” A: “nine”
  • Q: “What is 6+10?” A: “16”
  • Q: “What color is the sky?” A: “blue”
  • Etc.
