How to integrate fields with mailchimp

Hi!
I have a doubt with mailchimp integration.

I need fields like [name] [email] [telephone] to integrate the same fields inside Mailchimp. At this moment is working name,email. In builder have just fields for Name, Lastname and Email. I need a new field for phone. How I can customize this options?

I just make the for in Mailchimp, embed it and style it to my needs then…
