I blurred info that I am not sure is sensitive about the code link. I could be paranoid but that is why I blurred some of the code but it is just copy and paste from HoneyBook CRM

You see the code block on the right, location is in between the review and the map and on the left you see “execute code” is checked.

However on the live site the form is missing.

NOTES:
In chrome inspector the form isn’t rendered at all. Code goes from the review to the map.
I Use my hosts site cache flush
I also Purged my cache on my caching plugin

Still nothing.

any help is appreciated.

You need to sign that script or it will not work.
